SMS Opt-In Consent — How Patients Enroll

EFFECTIVE: AUGUST 13, 2026

This page documents, publicly and verbatim, how patients give consent to receive text messages through Klydmore Health (a product of RedArk Ventures LLC). There is no website sign-up form and no keyword-based enrollment: consent is collected in person by the patient's own healthcare practice during clinical enrollment, using the consent language shown below, and is documented in the patient's chart before any message is sent.

PATIENT CONSENT — TEXT-MESSAGE CARE PROGRAM (KLYDMORE HEALTH)

I consent to receive text messages from [Practice name], sent through Klydmore Health, at the mobile number I provide. These messages may include scheduled symptom questionnaires (such as the PHQ-9 and GAD-7), medication check-ins, reminders about those check-ins, and safety information approved by my practice.

I understand that: message frequency varies with the care plan my practice sets (typically several messages per week while a questionnaire is active); message and data rates may apply; I can reply HELP for help and STOP at any time to stop messages; consent is voluntary and not a condition of receiving care; and Klydmore is not an emergency service — in an emergency I will call 911, or call or text 988 for crisis support.
